The Greater Manchester Population Health Plan sets out an approach to delivering a radical upgrade in population health. It is informed by the best empirical evidence and by the views of the people of Greater Manchester. It aims to acknowledge the vital contribution communities make to health and wellbeing and put person and community-centred approaches at the centre of the plan.
