Greater Manchester VCSE Assembly: Health and Social Care: Population Health Plan - Thursday 26th October 2017

The Greater Manchester Population Health Plan sets out an approach to delivering a radical upgrade in population health. It is informed by the best empirical evidence and by the views of the people of Greater Manchester.
