Transitions Development Officer - Trafford CIL

£19600, pro rata, 28 hours (actual salary £15,680) - 18 month fixed term contract

Who we are:

Trafford Centre for Independent Living provides information, advocacy and brokerage to people with impairments across Trafford. We are a disabled people’s user-led organisation, set up using the expertise of disabled people to provide tailored support to people with impairments to live full, enjoyable and independent lives. This means enabling people to take responsibility for their own daily lives and decisions, and giving them the skills and confidence to play an active role in the care and the financial support that they receive.

The role:

Transition refers to the move from Children’s services (health, education and care) to Adult services. Transition is particularly important for young disabled people as making these changes can often be more complicated. The Transitions Development Officer will work directly with disabled children, young people and their families, as well as with providers of services to children and adults across Trafford, to support an integrated transition process for all involved. A key part of this will involve initial research and networking to establish the needs of families and providers, developing links between all providers and providing a clear signposting pathway into adult services.

You are:

An energetic, positive and forward thinking person, able to coordinate and develop our voluntary sector transition service, supporting young people to move smoothly from Children’s to Adult Services. You are a skilled communicator and negotiator, committed to partnership working to achieve demonstrably good outcomes for young people with disabilities, with a solid understanding of self-directed support principles and processes.

The small print:

The role is 28 hours per week, with occasional evening work anticipated. We operate remote and flexible working options, with excellent annual leave entitlement of 28 days, plus a 3% contributory pension scheme.
